Form I-9 — Employment Eligibility Verification (Section 1)

Federal law requires all U.S. employers to verify the identity and work authorization of every new hire using Form I-9. You are completing Section 1 below — the portion of the form completed by the employee.

Before You Begin

  • Please review the official USCIS Form I-9 Instructions (opens in a new tab).
  • Complete every required field. Do not leave fields blank — enter “N/A” if a field does not apply to you.
  • Use your full legal name as it appears on your government-issued identification.
  • Knowingly providing false information on Form I-9 is a federal crime punishable by fines and imprisonment.

What Happens After You Submit

  • Your completed Section 1 will be saved and stored securely as part of your hiring record.
  • Madalyn Whitsett (HR) will contact you to schedule Section 2, where your identity and work authorization documents will be verified — either in person at our corporate office, or via a live video call under DHS’s E-Verify alternative procedure.
  • Section 2 must be completed within 3 business days of your first day of work.
  • Have your acceptable identification documents ready (see the list below).

Section Break

Acceptable Documents for I-9 Section 2

For Section 2, you will need to present either ONE document from List A, OR ONE document from List B AND ONE document from List C. All documents must be unexpired originals (or certified copies, where indicated). Photocopies are not acceptable for in-person verification.

List A — Documents That Establish BOTH Identity and Work Authorization

If you provide a document from List A, you do not need List B or List C documents.

  • U.S. Passport or U.S. Passport Card
  • Permanent Resident Card or Alien Registration Receipt Card (Form I-551, “Green Card”)
  • Foreign passport with a temporary I-551 stamp or printed notation on a machine-readable immigrant visa
  • Employment Authorization Document (EAD) that contains a photograph (Form I-766)
  • For nonimmigrant aliens authorized to work for a specific employer: foreign passport with Form I-94 or Form I-94A bearing the same name and an endorsement of the alien’s nonimmigrant status, as long as the period of endorsement has not expired and the proposed employment is not in conflict with restrictions or limitations identified on the form
  • Passport from the Federated States of Micronesia (FSM) or the Republic of the Marshall Islands (RMI) with Form I-94 or I-94A indicating nonimmigrant admission under the Compact of Free Association

List B — Documents That Establish IDENTITY Only

You must provide ONE document from List B AND ONE document from List C.

  • Driver’s license or ID card issued by a U.S. state or outlying possession (must contain a photograph or identifying information such as name, date of birth, gender, height, eye color, and address)
  • ID card issued by federal, state, or local government agencies (with photograph or identifying information)
  • School ID card with photograph
  • Voter registration card
  • U.S. Military card or draft record
  • Military dependent’s ID card
  • U.S. Coast Guard Merchant Mariner Card
  • Native American tribal document
  • Driver’s license issued by a Canadian government authority

For minors under age 18 only (who cannot present a List B document above):

  • School record or report card
  • Clinic, doctor, or hospital record
  • Day-care or nursery school record

List C — Documents That Establish WORK AUTHORIZATION Only

You must provide ONE document from List B AND ONE document from List C.

  • U.S. Social Security Account Number card (unless it specifies “NOT VALID FOR EMPLOYMENT”)
  • Certification of Report of Birth issued by the U.S. Department of State (Form FS-545, Form DS-1350, or Form FS-240)
  • Original or certified copy of a birth certificate issued by a U.S. state, county, municipal authority, or territory bearing an official seal
  • Native American tribal document
  • U.S. Citizen ID Card (Form I-197)
  • Identification Card for use of Resident Citizen in the United States (Form I-179)
  • Employment authorization document issued by the Department of Homeland Security (other than those listed under List A)

Important: All documents must be unexpired and must reasonably appear to be genuine and to relate to you. Receipts for replacement documents are acceptable in limited circumstances — see the USCIS Form I-9 Instructions for details.
